GENERAL INSTRUCTIONS FOR ALL NOMINATIONS
Type or print clearly. It is not necessary to use the forms, but be sure to include all the information. Please provide the person or chapter making the nomination, otherwise your nomination may be eliminated.
Be very specific with the reasons.
Mail all nominations to:

Mail them before August 15, 2012.
Mark on the lower left corner of the envelope, the type of nomination enclosed.
These will only be opened by out of state guests.
We will accept nominations at the Samboree ONLY on Friday, August 31, before NOON. You may give them to either Jim or Judie.
Do not mail books or pictures, give them to Jim or Judie at the Samboree, by noon on Friday.
You may make copies of any form!!! You may use extra pages.
